A treatment we approach with caution

Tear-trough filler is one of the most popular and one of the most poorly performed treatments in the UK aesthetics industry. It is the most common reason clients arrive at Aesthetic Clinique for hyaluronidase dissolution, often with chronic under-eye puffiness, blue tints, lumpiness or a face that looks subtly unwell rather than refreshed.

We perform tear-trough filler — but only when we believe the anatomy genuinely suits it, the cause of the under-eye concern is genuinely correctable with filler, and the client has realistic expectations. This means we turn down a meaningful percentage of enquiries, and recommend alternatives in many of those.

Why dark circles are not all the same

Dark circles have multiple causes that look superficially identical and respond very differently to filler:

  • Shadow from true hollowing — filler can help
  • Pigmentation (often genetic, common in clients with darker skin tones) — filler does nothing; topical or laser treatment is the right approach
  • Visible underlying veins or muscle — filler may worsen the appearance via the Tyndall effect, where blue tones show through thin skin
  • Fat-pad bulging ("eye bags") — filler makes this worse, not better; surgical assessment is appropriate

We assess all four in consultation and recommend the right tool, which is sometimes not filler at all.

Who tear-trough filler suits

The best candidates are typically clients aged 25–50 with true bony tear-trough hollowing (a visible groove along the orbital rim), reasonably thick lower-eyelid skin, no significant fat-pad herniation, and shadow as the dominant cause of their dark circles. They have realistic expectations of partial improvement, not perfection.

The wrong candidates — clients with prominent eye bags, very thin skin, pigmented circles, or unrealistic expectations — are common, and we will say so.

On the day at Aesthetic Clinique

A tear-trough appointment takes around 40 minutes. We use a blunt cannula in essentially all cases — the under-eye contains critical vasculature and cannula technique substantially reduces vascular risk. Soft, low-cohesivity HA filler is placed deep on the bony orbital rim in very small aliquots. We treat one side, reassess, then treat the other.

Mild swelling and occasional bruising are normal. Settling is slower than other areas — full settling is typically at 3–4 weeks rather than two. We review at two weeks regardless.

Longevity and reversibility

Tear-trough filler often lasts 12–18 months or longer because the area moves very little. If the result is not what you wanted, hyaluronidase fully reverses HA filler in this region. We treat dissolving here with particular care — see our hyaluronidase service for detail.

If we don't think filler is right for you, we will tell you. Sometimes the best answer is cheek filler instead, sometimes a topical pigmentation protocol, sometimes polynucleotides for skin quality, and sometimes no treatment at all.

    Detailed Suitability Assessment

    We assess skin thickness, fat-pad position, presence of true hollowing vs pseudo-hollowing, vascularity and pigmentation. Many clients are advised against treatment at this stage.

    Marking, Numbing & Vasculature Check

    Specific entry points are marked. Topical numbing is applied. The under-eye region has critical vasculature, so cannula technique is the default.

    Deep Placement on Orbital Rim

    Soft, low-G' HA filler is placed deep on the bony orbital rim using a blunt cannula, in very small aliquots. We treat one side at a time and reassess between sides.

  4. 4

    Two-Week Review

    Swelling settles slowly under the eye. We see you back at two weeks before deciding on any small top-up. Most clients need no further treatment.

May not be suitable if

  • Significant fat-pad herniation or 'eye bags' — surgical assessment is more appropriate
  • Pigment-driven dark circles (treatable, but not with filler)
  • Very thin lower-eyelid skin where filler is likely to show as a blue tint (Tyndall effect)
  • Clients with previous tear-trough filler placed superficially that has not been dissolved
  • Pregnancy, breastfeeding, or active eye-area infection

Aftercare

Caring for your results

A few simple steps in the first 24–48 hours help your results settle as expected.

  • Avoid pressing, rubbing or massaging the under-eye for 72 hours
  • Sleep elevated on at least two pillows for the first two nights
  • Skip strenuous exercise, alcohol and very salty food for 48 hours
  • Use only mineral SPF on the area; no makeup for 12 hours
  • Expect prolonged mild puffiness for up to two weeks
  • Avoid facials, eye-area treatments and saunas for one week

Tear-trough filler is one of the most under-regulated and over-performed treatments in the UK aesthetics industry, and the most common reason clients come to us for hyaluronidase dissolution. We use only MHRA-regulated HA filler products specifically formulated for the under-eye region — soft, low-G', cohesive gels designed for deep placement. Treatment is fully reversible with hyaluronidase. We refuse treatment in clients whose anatomy makes complications likely.

Frequently Asked Questions

Is tear-trough filler the right treatment for my dark circles?

Sometimes — and often not. Dark circles have at least three causes: shadow from hollowing (correctable with filler), pigmentation (filler will not help), and visible underlying vasculature (filler may worsen this). We will only recommend filler if shadow from genuine hollowing is the dominant cause, and we will tell you honestly when it isn't.

Why do tear-trough fillers sometimes look bad?

Common problems are filler placed too superficially (creating lumpiness or a bluish 'Tyndall' tint), too much volume (creating chronic puffiness), or treatment in clients whose anatomy was never suitable. All are correctable with hyaluronidase, but the better answer is selecting the right candidate in the first place — which is why we turn many enquiries down.

How long does tear-trough filler last?

Often longer than other areas — sometimes 12–18 months or more — because the under-eye region moves very little. This is a double-edged feature: a good result lasts well, but a result you don't like also lasts well unless dissolved.

Will I see results immediately?

Partial improvement is visible immediately. Final settling takes longer than other filler areas because the under-eye is prone to mild persistent swelling — full settling is typically at three to four weeks rather than two.

Can tear-trough filler be dissolved if I don't like it?

Yes. Hyaluronidase fully reverses HA tear-trough filler. We treat dissolving in this area with particular care — small doses, careful placement — because over-dissolution can deflate surrounding mid-face filler. Many clients we dissolve come from other clinics and find dissolving alone significantly improves their appearance.

Why might you turn me down for tear-trough filler?

Common reasons: significant fat-pad bulging (filler makes this worse), very thin lower-lid skin (high risk of blueness), pigment-driven dark circles (filler does nothing for these), or anatomy where cheek filler would deliver a better under-eye result without the risk. We will explain in detail and recommend an alternative if there is one.

Should I have cheek filler instead?

Often, yes. Restoring volume to the upper cheek often softens the tear-trough indirectly by lifting surrounding tissue, with much lower technical risk. For clients with combined mid-face and tear-trough volume loss, treating the cheek first is usually the right starting point — and sometimes the only treatment needed.
