A high-impact, often-overlooked treatment

Hollowed temples are one of the earliest visible signs of facial ageing — and one of the most under-recognised. Most clients arriving at Aesthetic Clinique have not noticed their own temple hollowing in the mirror, but it is often the change that makes the upper face look "tired" or "unwell" while the rest of the face looks essentially unchanged.

A small, well-placed dose of firm HA filler in the temples often achieves more for overall facial freshness than a much larger investment in any single mid-face area. It is one of the treatments we recommend most often to clients who have arrived asking for something else.

Why temples hollow

The temple region (the temporal fossa) sits between the lateral brow and the hairline. It contains layered muscle, fat pads and bone — and all three thin with age. Bone resorbs along the lateral orbital rim, the deep temporal fat pad atrophies, and the temporalis muscle thins. The result is a visible hollow that casts shadow on the lateral upper face, drops the brow tail and disrupts the smooth contour from forehead to cheek. Significant weight loss and prolonged stress accelerate the process.

What temple filler can — and can't — do

Temple filler restores the volume that bone, fat and muscle have lost. Done well, it lifts the lateral brow, softens the upper face, and supports the cheek and jaw indirectly. It does not address heavy upper-eyelid skin (which may need surgical assessment), it does not remove forehead lines (which respond to facial line treatment), and it cannot meaningfully change the position of the medial brow.

We will not treat clients with naturally flat, non-hollowed temples seeking exaggerated lateral definition — this is a misuse of the area and risks unnatural results.

On the day at Aesthetic Clinique

A temple appointment takes around 40 minutes. We map the vasculature carefully, mark a single safe entry point on each side, and apply topical numbing. A blunt cannula is introduced and firm HA filler is placed deep in the temporal fossa, beneath the temporalis muscle, in small controlled volumes. Cannula technique is the strong default in this area for vascular safety.

Most clients experience only mild bruising at the entry point and minimal swelling. Results are visible immediately and settle fully within two weeks.

Longevity and combinations

Temple filler typically lasts 12–18 months. It combines particularly well with cheek filler as part of a comprehensive volume-restoration plan, and with subtle facial line treatment for clients whose forehead and brow lines also contribute to an "aged upper face" appearance. We always plan in stages — temples first for many clients, with mid-face added at later sessions if needed.

Caring for your results

A few simple steps in the first 24–48 hours help your results settle as expected.

  • Avoid pressing or massaging the temples for 48 hours
  • Skip strenuous exercise, alcohol and saunas for 24–48 hours
  • Sleep on your back for the first two nights
  • Avoid wearing tight headwear, glasses on the temple area, or headphones for 24 hours
  • Mild bruising at the entry point is normal and may last a week
  • Avoid scalp treatments and hair colouring for 48 hours

We use only MHRA-regulated, hyaluronic-acid fillers — firmer products designed for deep, structural placement. Treatment is performed by cannula in essentially all cases due to the temple's vascular anatomy. HA filler is fully reversible with hyaluronidase. The temple is one of the higher-risk filler areas anatomically; we discuss vascular risk explicitly in consultation and consent.

Frequently Asked Questions

Why do temples become hollow?

Temple hollowing is caused by a combination of bone resorption, fat-pad atrophy and muscle thinning that begins early — often visible from the late 30s. Significant weight loss accelerates it. Hollow temples cast shadows and visibly age the upper face, even on otherwise youthful skin.

How much filler do temples need?

Most clients need 1–2ml per temple. Very deep hollowing may need 2–3ml per side staged across two sessions. We dose conservatively in the first session and review at two weeks before considering additional volume.

How long does temple filler last?

Typically 12–18 months. Because the temples move very little and we use firmer HA products designed for deep placement, longevity is at the upper end of HA filler ranges.

Is temple filler safe?

Temple filler is safe when performed correctly with cannula technique by a qualified clinician who understands the vascular anatomy of the area. The temple does contain important vessels and we discuss vascular risk explicitly. We do not treat clients who would not accept this risk after detailed counselling.

Will temple filler change my hairline?

No. Temple filler is placed deep beneath the temporalis muscle in the temporal fossa, well away from the hairline. It does not affect hair growth, hair density or hairline position.

Can temple filler help with sagging brows?

Indirectly, yes. Restoring temple volume provides lateral support to the brow tail and creates the appearance of a subtly lifted, more open upper face. For clients with brow descent caused primarily by muscle action rather than volume loss, facial line treatment is the more appropriate first step.

Does temple filler hurt?

Most clients tolerate it well. We use a single cannula entry point per side rather than multiple needle sticks, topical numbing is applied first, and the filler contains lidocaine. The most common discomfort is mild pressure during placement, which settles immediately.
