Treatment Summary

Sculptra is an injectable poly-L-lactic acid bio-stimulator that triggers your skin's own collagen production over months rather than placing immediate volume. Treatment is delivered as a course of 2–3 sessions spaced 4–6 weeks apart, with results developing gradually over 3–6 months and lasting up to 18–24 months. It is a structural skin-rebuilding tool, not a quick fix.

Key Benefits

Rebuilds Structural Collagen

Stimulates the skin's own type I collagen — the structural collagen that supports the face.

Gradual, Natural Result

Improvement develops over 3–6 months. No instant change, no sudden new face.

Long-Lasting

Effects typically last 18–24 months — significantly longer than hyaluronic acid filler.

Restores Foundation, Not Surface

Addresses underlying tissue thinning rather than only treating the visible line or hollow.

What Sculptra is

Sculptra is an injectable made from poly-L-lactic acid (PLLA) — a biocompatible, biodegradable polymer that has been used in dissolvable surgical sutures for decades. In aesthetic use, it is reconstituted into a suspension and injected into the deep dermis or subdermal plane. Once placed, it is gradually broken down while triggering the skin's own production of type I collagen — the structural collagen that gives the face its underlying scaffolding.

It is not a filler. There is no immediate volume effect. The improvement you eventually see is your own tissue, rebuilt over months.

How it works

PLLA microparticles act as a controlled stimulus to fibroblasts. Over a period of roughly 6–12 weeks following each treatment, fibroblasts respond by producing new collagen. By the third or fourth month, this new collagen is producing visible improvements in firmness, contour and skin density. By month six, the result is at its peak. The PLLA particles themselves are gradually metabolised over many months, but the collagen they induced remains long after they are gone.

Who Sculptra suits

Sculptra is best for clients in their mid-30s and beyond with generalised volume loss — the kind of facial deflation where multiple syringes of hyaluronic acid filler would be required to address it. It is excellent for the cheeks, temples, jawline support and décolletage. It is not the right tool for the lips, tear troughs or other dynamic/superficial areas — those need different tools.

It is also a good choice for clients who specifically want fewer, longer-lasting interventions rather than yearly filler top-ups. The trade-off is that you must be patient: there is no instant gratification.

Who Sculptra does not suit

Sculptra is not appropriate during pregnancy or breastfeeding, in clients with active autoimmune or connective-tissue disease, or with a history of keloid scarring or hypertrophic granuloma formation. It is also the wrong choice if you want a visible result for an event in two weeks — the result simply does not work that way.

What happens on the day

After consultation and planning, the Sculptra is prepared (typically pre-reconstituted at least 24–72 hours in advance per current protocols, then mixed with anaesthetic on the day). We deliver it via cannula or needle into the planned planes, usually 1–2 vials per session depending on volume loss. The treatment takes around 45–60 minutes. We finish with thorough massage and we will ask you to follow the '5-5-5' rule at home: massage 5 minutes, 5 times a day, for 5 days.

Results timeline

Immediately post-treatment you will see mild swelling — this is the water in the suspension, not your result. It reabsorbs within days. Over the following weeks and months, your own collagen response builds. We formally review at 3 months and 6 months, and a second or third session may be planned around those reviews.

Maintenance and combinations

Most clients return for a single maintenance session every 18–24 months. Sculptra often combines well with Profhilo (skin quality), Radiesse (hands or specific contour), or a small amount of filler for precise definition once the structural rebuild is well underway.

What to Expect

  1. 1

    Consultation and Planning

We assess facial volume, skin quality and ageing pattern, and confirm Sculptra is the right tool versus filler or Profhilo.

  2. 2

    Reconstitution

Sculptra is reconstituted with sterile water at least 24–72 hours before injection (per current best-practice protocols) and prepared with anaesthetic on the day.

  3. 3

    Injection

Delivered via cannula or needle into deep dermal/subdermal planes across the cheeks, temples and jawline as planned. Massage is performed at the end.

  4. 4

    Course and Follow-Up

Typically 2–3 sessions 4–6 weeks apart. Results are formally reviewed at 3 and 6 months.

Suitability

Aesthetic care is highly individual. The points below are general guidance — final suitability for sculptra collagen induction is always confirmed in your consultation.

Often suitable for

  • Adults 35+ with generalised facial volume loss and bone-level thinning
  • Clients seeking long-lasting structural rejuvenation without yearly filler top-ups
  • Temples, cheeks and lower-face support where multiple syringes of filler would otherwise be needed
  • Post-weight-loss facial deflation
  • Décolletage skin laxity (off-face protocols)

May not be suitable if

  • Clients wanting immediate visible change for an event
  • Active autoimmune or inflammatory connective-tissue disease
  • History of keloid scarring or hypertrophic granuloma formation
  • Pregnancy or breastfeeding
  • The lips, tear troughs and other dynamic/superficial areas (different tools used)

Aftercare

Caring for your results

A few simple steps in the first 24–48 hours help your results settle as expected.

  • Massage the treated area for 5 minutes, 5 times a day, for 5 days (the '5-5-5' rule)
  • Avoid strenuous exercise, saunas and facial massages for 24–48 hours
  • Avoid alcohol and blood-thinning supplements for 24 hours
  • Mild swelling and tenderness for 24–72 hours is normal
  • Photograph at 3 and 6 months — daily change is too gradual to perceive

Sculptra (poly-L-lactic acid) is FDA-approved and CE-marked, with over 20 years of clinical use, originally for HIV-related lipoatrophy and subsequently for cosmetic facial volume loss. Peer-reviewed evidence supports neocollagenesis (production of new type I collagen) over 3–6 months post-injection, with effects sustained up to 24 months. It carries a well-characterised safety profile when delivered with appropriate reconstitution and injection technique.

Frequently Asked Questions

How is Sculptra different from filler?

Filler places a hyaluronic acid gel that adds volume immediately and lasts 9–18 months. Sculptra places a poly-L-lactic acid microparticle that does not add volume by itself — instead, it triggers your skin to produce its own collagen over 3–6 months. The result lasts longer (up to 24 months) and looks more like an improvement in your underlying tissue rather than 'something added'.

When will I see results?

You will see initial mild swelling immediately, but this is water — not the result. The water is reabsorbed within days, and over the following 3–6 months your own collagen response gradually improves volume, firmness and contour. We formally review at 3 months and again at 6 months.

How many sessions will I need?

Most patients need 2–3 sessions, spaced 4–6 weeks apart. The number depends on the degree of volume loss and the area being treated. We make this assessment at consultation and may revise after the first session response.

How long do results last?

Up to 24 months for most clients. Maintenance is usually a single top-up session every 18–24 months. Because the result is your own collagen, the underlying improvement does not disappear instantly when maintenance is delayed — it tapers off.

Are there risks of nodules or lumps?

Late-onset nodules are the most discussed risk with Sculptra historically. With current reconstitution protocols (longer reconstitution time, larger dilution volumes), correct injection plane and the '5-5-5' massage routine, this risk is markedly reduced. We follow current best-practice protocols and brief you fully.

Can Sculptra and filler be used together?

Yes — and they often are. Sculptra rebuilds underlying volume; filler can be used for precise structural definition (e.g. chin projection) or fine areas Sculptra cannot reach (lips, tear troughs). We sequence carefully, usually starting with Sculptra and then refining with filler at the 3-month review.

Is Sculptra safe for darker skin tones?

Yes. Sculptra has a good safety profile across Fitzpatrick types and does not carry the post-inflammatory hyperpigmentation risk associated with energy-based treatments. Injection technique still matters, and we will discuss bruising risk individually.
