What cheek filler is — and isn't

Cheek filler is volume restoration, not surgical lifting. It cannot meaningfully address heavy skin laxity, deep static jowls, or significant lower-face descent — these often require surgical or skin-tightening intervention. It also is not the right treatment for anyone seeking a strongly sculpted, high-projecting "model cheekbone" look on a face that does not naturally have that bone structure. We design treatment around your face, not around an aesthetic borrowed from social media.

How we plan cheek treatment

In consultation we assess three things: bone projection (your underlying structure), volume loss (how empty the deep fat compartments are), and skin quality (how the overlying tissue will respond to support). We then decide where to place filler — deep on bone for projection, into the deep medial cheek for support, or laterally for contour — and how much. Most first treatments use 1–2ml in total. We always prefer to review and add at two weeks rather than treat to maximum volume in one session.

On the day at Aesthetic Clinique

A cheek appointment takes around 40 minutes including numbing, marking and treatment. We use a blunt cannula wherever possible — it reduces bruising and significantly lowers vascular risk. Filler is placed in small, controlled boluses on the periosteum (bone surface) or into the deep cheek fat. You will see results immediately, with a small amount of swelling for 24–48 hours.

Longevity and follow-up

Cheek filler is one of the longest-lasting HA areas — typically 12–18 months — because the area moves less and the product used is firmer. Most clients return for a small maintenance dose at 12–18 months rather than a full repeat treatment. Cheek work combines naturally with chin and jawline filler for clients addressing overall lower-face proportion, and with subtle tear-trough treatment in suitable candidates.

A few simple steps in the first 24–48 hours help your results settle as expected.

  • Avoid pressing or massaging the cheeks for 48 hours
  • Skip strenuous exercise, alcohol and saunas for 24–48 hours
  • Sleep on your back for the first two nights to avoid displacement
  • Avoid facial treatments and dental work for one week
  • Use mineral SPF and avoid intense heat for 48 hours
  • Mild bruising and tenderness for up to a week is normal

HA filler is fully reversible with hyaluronidase. We do not use permanent or biostimulator products in clients seeking pure HA filler — biostimulators (Sculptra, Radiesse) are discussed separately as a different category of treatment.

How much cheek filler will I need?

Most cheek treatments use 1–2ml in total across both sides for a first session. Significant volume loss may need 2–4ml staged over two appointments. We dose conservatively in the first session and review at two weeks before adding more — never the maximum in one go.

How long do cheek fillers last?

Cheek filler typically lasts 12–18 months. Because the cheek area moves less than the lips and we use firmer HA products designed for deep placement, longevity is at the upper end of HA filler ranges. Maintenance is usually annual for those wishing to preserve the result.

Will cheek filler give me a 'pillow face' look?

Not when dosed appropriately and placed deep on bone. The over-filled mid-face look comes from repeated, superficial volume placed over many sessions. We place filler deep, dose conservatively, and refuse treatment requests that would compromise facial proportion.

Can cheek filler help with nasolabial folds?

Often, yes — and usually more effectively than treating the folds directly. The nasolabial fold is partly caused by mid-face volume loss; supporting the cheek lifts surrounding tissue and softens the fold without flattening expression. We will discuss in consultation which approach suits you.

Is the treatment painful?

Most clients describe cheek filler as easily tolerable. Topical numbing is applied, the HA filler contains lidocaine, and we generally use a blunt cannula which reduces both pain and bruising. Most clients return to normal activities the same day.

What is the difference between cheek filler and a thread lift?

Cheek filler restores volume; thread lifts reposition tissue. They address different problems. For most clients with age-related volume loss, cheek filler is the more appropriate treatment. Significant skin laxity may need surgical assessment rather than either intervention.

Can I have cheek filler if I have had Sculptra or Radiesse?

Usually yes, but timing matters. We assess any previous biostimulator work in consultation. HA filler is generally placed in a different plane to biostimulators, but we always review your full treatment history first.
